This form is to request budgeted funds and reimbursement from the PTA. See Budget tab for detailed line items. If there are any questions, please contact the Treasurer at sacpta@yahoo.com before spending funds.
TEACHERS AND STAFF: PTA has budgeted funds for classrooms, grade level teams, and departments. PTA funds must be used to benefit Sacajawea Elementary students’ health, welfare, safety, or education. Grade level team funds can be used by the entire team for one or more purpose(s) or the team can decide to divide them equally among their classrooms. Each grade level team should notify PTA in how they have decided as a team to use these funds. An email to sacpta@yahoo.com will suffice. If funds are to be granted to VSD or Foundation for VPS, see the school secretary.

Cash Box

Every PTA at some time will need cash for an event. In PTA Money Matters it suggests a Cash Box form to help with this process. The Treasurer will write a check to the individual (payee) not for cash, who is responsible for getting the cash for the cash box. As with all checks, two signatures are required. If it is the Treasurer who will get the cash, it is recommended that two different signers sign the check.

Please submit this form with any money passed to the Treasurer. Insurance requires that two PTA members must count the funds, sign this form, and turn in with funds.
Unbudgeted Funds Request

This form is to request unbudgeted funds from the PTA. (Funds in budgeted categories are paid through the Expense Report/Check Request.) Your request will be reviewed at the next General Meeting and will get a yes or no vote, unless further information or consideration is requested by the PTA. Please allow ample time for this process. Attach any research you have done on cost and include enough detail about how the funds will be spent to ensure the board/members has sufficient information to reach a decision. PTA funds must be used to benefit Sacajawea Elementary students’ health, welfare, safety, or education and your request will be considered in light of those requirements.

Please submit this form with any cash box money passed to the Treasurer. Insurance requires that two PTA members must count the money, sign this form, and turn in with funds. The Treasurer will attach a copy of this Deposit to the original Cash Box Funds Request as documentation that the cash box funds were replaced after the event.
